refactor(Provider): remove deprecated provider/bind API (#10652)

Closes #9751

BREAKING CHANGE:

These forms of providers are no longer accepted:
  bind(MyClass).toFactory(...)
  new Provider(MyClass, toFactory: ...)

We now only accept:
  {provider: MyClass, toFactory: ...}
