refactor(compiler): remove unused code

BREAKING CHANGE:
- Removes `ChangeDetection`, use a binding for `ChangeDetectorGenConfig` instead
  to configure change detection.
- `RenderElementRef.renderBoundElementIndex` was renamed to `RenderElementRef.boundElementIndex`.
- Removes `ViewLoader`, use `XHRImpl` instead.
